صحة: دراسة سريرية تؤكد فاعلية دواء جديد للزهايمر

أعرب عدد من الباحثين عن ارتياحهم إلى النتائج التفصيلية لدراسة سريرية عن دواء جديد أكدت فاعليته في إبطاء التدهور المعرفي لمرضى ألزهايمر التنكسي العصبي، لكنهم حذروا في الوقت ذاته من بعض الآثار السلبية التي قد تنجم عن استخدام العلاج.

وأكدت النتائج الكاملة للدراسة السريرية المتقدمة (المرحلة الثالثة) التي أجريت على نحو 1800 شخص تمت متابعتهم على مدار 18 شهرا، انخفاضا بنسبة 27% في التدهور المعرفي لدى المرضى الذين عولجوا بدواء "ليكانيماب" (Lecanemab) الذي توصلت إليه مجموعة الأدوية اليابانية "إيساي" (Eisai) والأميركية "بايوجين" (Biogen).

وسبق أن اُعلن في نهاية سبتمبر الماضي عن هذه النسبة التي تُعتبر "مهمة إحصائيا" بحسب المجموعتين.

لكن الدراسة الكاملة التي نُشرت الأربعاء 30 نوفمبر 2022 في مجلة "نيو إنغلند جورنا أوف ميديسين"، بينت أيضا معدلات تسجيل الآثار الجانبية لعقار "ليكانيماب"، وهي تكون أحيانا حادة وأكثر تواترا بشكل ملحوظ من مجموعة المرضى الذين تناولوا الدواء الوهمي.

وتبيّن أن 17.3% من المرضى الذين تلقوا "ليكانيماب" عانوا نزفا دماغيا، في حين اقتصرت النسبة على 9% بين الذين تناولوا الدواء الوهمي.

وعانى 12.6% من الأشخاص الذين تلقوا هذا الدواء التجريبي من وذمة دماغية، مقارنة بـ 1.7% فحسب في مجموعة الدواء الوهمي.

غير أن معدل الوفيات الإجمالي هو نفسه تقريبا لدى مجموعتي المرضى في الدراسة (0.7% لدى الأشخاص الذين عولجوا بـ "ليكانيماب"، و0.8% لدى أولئك الذين عولجوا بدواء وهمي).

وقال مدير معهد الأبحاث المتعلقة بالخرف في المعهد البريطاني بارت دي ستروبر، إن "هذا هو الدواء الأول الذي يقدم خيارا علاجيا حقيقيا للأشخاص المصابين بمرض ألزهايمر. رغم أن الفوائد السريرية تبدو محدودة نوعا ما، فإن من المتوقع أن تصبح أكثر وضوحا إذا تم تناول الدواء لفترة أطول".

ويعاني مرضى ألزهايمر من لويحات بروتينية تسمى "أميلويد" تتشكل حول الخلايا العصبية وتدمرها في النهاية، ويتراكم بروتينان رئيسيان هما تاو وبيتا أميلويد بشكل غير طبيعي في الدماغ، مما يتسبب في موت خلاياه وتقلصه.

ومن أبرز نتائج ذلك فقدان الذاكرة وعدم القدرة على أداء المهام اليومية. ويعتبر هذا المرض من أهم مشاكل الصحة العامة إذ يصيب أكثر من 40 مليون شخص في كل أنحاء العالم.

ويستهدف عقار "ليكانيماب" رواسب بروتين بيتا أميلويد ولكن فقط في المراحل المبكرة من مرض ألزهايمر، مما قد يحد من استخدامه لأن هذا المرض غالبا ما يُشخص في وقت متأخر.
